By registering , you, the parent/guardian of the child , give them permission to participate in the 2023 VBS at "Iglesia Bautista La Gracia" 19650 SR 44 Eustis. You release the church and its representatives from any liability in the event of an accident en route, during or returning from the VBS. I also authorize you to obtain any emergency medical care that may be required during my son's/daughter's attendance. I hereby grant permission for Iglesia Bautista La Gracia to record sounds, images, or video of my children while attending this 2022 VBS program. I also give permission for Iglesia Bautista La Gracia at its sole discretion, to use these sounds, images, or videos in publications (including print, websites, and social media platforms) owned by Iglesia Bautista La Gracia Church in relation to this 2023 VBS program.
